The Politics of Institutional Reform

What are some examples of poorly performing institutions in other policy realms, outside of education, that are difficult to reform because of vested interest power?

Congress for starters. Outside the realm of our most powerful government institutions, the most immediate that comes to mind is our prison industrial complex. Obvious changes continue to not happen because of the profits that can be made by the power and finances of vested interests.
